Requesting or Editing a Custom Form

If I need more advanced help with making or editing a custom form, how can I request support?

We have many resources to help you with making a form (please see our Building & Editing Forms collection).

In some cases, you may need our team to help you with that build or update/fix to an existing form. If you are working on updating multiple forms, submit separate requests for each form to ensure accuracy and efficiency.

If that is your case, you can follow the steps below to submit your form request.

Click the Chat Icon

Locate the chat icon on the bottom right-hand side of your screen and click on it.

Select “Custom Form Request”

Under “Create a Ticket,” choose “Custom Form Request.”

Fill Out the Form

You’ll be prompted to complete a short form. Please include the following details:

  • Request Type: Select whether this is a Fix, Update, or New form request.

  • Form Name: Enter the exact name of the form you're referencing.

  • Expedite Option: Indicate if you'd like the form expedited for an additional fee.

  • Subdomain: Provide the subdomain where the form should be placed.

  • Multiple Forms: If you are updating multiple forms, create a separate request for each form to streamline the process and reduce errors. If you require expedited processing for your form request, you can select the "Expedite Option" during the ticket creation process. Note that this may incur an additional fee.

Once your forms are complete, our Forms Coding Manager will reach out to let you know that they are ready for you to test and assign. Before assigning, please test the functionality to ensure all fields have been correctly implemented and coded. Ensure that all fields are functioning as intended and that the form aligns with your requirements. If any issues are identified, contact the forms team for further assistance.
